Der übermäßige Einsatz von Antibiotika, sowohl in der Human- als auch in der Veterinärmedizin, fördert hierbei die Entstehung der Antibiotikaresistenzen. Dabei steht fest, dass Antibiotikaresistenzen nicht gänzlich vermieden werden können. Daraus resultierend, sinkt die Verfügbarkeit an Therapieoptionen bei Infektionen mit bakteriellen Erregern und die Krankheitslast für Menschen und Tiere steigt. Mit einer Gesamtzahl von 26,4 Millionen Tieren, stellen Hunde und Katzen in Deutschland einen nicht unerheblichen Anteil an Lebewesen dar, die mit Antibiotika behandelt werden. Aus diesem Grund widmet sich die vorliegende Forschungsarbeit den Antibiotikaresistenzen bei Hunden und Katzen und soll, unter Anwendung des One Health-Ansatzes, geeignete Handlungsempfehlungen zur Bekämpfung der Antibiotikaresistenzen bei diesen Tiergruppen darbieten. Methodik – Zur Beantwortung der Forschungsfrage wird eine methodische Triangulation durchgeführt. Hierfür erfolgt eine systematische Literaturrecherche in der Fachdatenbank PubMed und der Suchmaschine Google Scholar. Zusätzlich wird eine Portalsuche auf den Internetseiten verschiedener Institutionen mit einer anschließenden Dokumentenanalyse durchgeführt. Außerdem werden drei leitfadengestützte Expert*inneninterviews geführt, deren Auswertung anhand einer computergestützten qualitativen Inhaltsanalyse erfolgt. Ergebnisse – Im Rahmen dieser Forschungsarbeit konnten diverse Handlungsempfehlungen aus verschiedenen Bereichen erarbeitet werden. Übergeordnet gilt es dabei, einen rationalen Antibiotikaeinsatz zu fördern. Zusammenfassend sollten Hygienemaßnahmen umgesetzt werden, eine Anpassung der Rahmenbedingungen erfolgen, Fach- und Privatpersonen nachhaltig aufgeklärt, eine kontinuierliche Datenerhebung umgesetzt sowie diagnostische Methoden angepasst werden. Da eine Verantwortung bei allen Beteiligten gesehen wird, bedarf es vor allem einem multidisziplinären Vorgehen und der Kombination verschiedener Maßnahmen, um eine kumulative Wirkung zu erzielen. Hierfür hat sich das Antibiotic Stewardship als passendes Mittel herausgestellt. Fazit – Zukünftig müssen umfassende Daten zu Erregern, Resistenzen und Übertragungsdynamiken bei Hunden und Katzen gesammelt werden. Mithilfe dieser, können wirksame und evidenzbasierte Antibiotic Stewardship-Maßnahmen erarbeitet werden, welche die Umsetzung eines rationalen Antibiotikaeinsatzes fördern sollen. Der Einbezug aller Beteiligten ist hierfür unabdingbar und festigt die Relevanz des One Health-Ansatzes.

Background – Antibiotic resistance is a global health risk for humans and animals. The excessive use of antibiotics, both in human and veterinary medicine, promotes the development of antibiotic resistance. It is evident that antibiotic resistance cannot be completely prevented. As a result, the availability of treatment options for infections with bacterial pathogens is decreasing and the burden of disease for humans and animals is increasing. With a total number of 26.4 million animals, dogs and cats in Germany represent a not insignificant proportion of living beings that are treated with antibiotics. For this reason, the present research work is dedicated to antibiotic resistance in dogs and cats and, using the One Health approach, is intended to present suitable recommendations for action to combat antibiotic resistance in these animal groups. Method – To answer the research question, a methodological triangulation is carried out. For this purpose, a systematic literature search is conducted in the PubMed database and the Google Scholar search engine. In addition, a portal search on the websites of various institutions with a subsequent document analysis will be conducted. Furthermore, three guided interviews with experts will be conducted, which will be evaluated by means of a computer-assisted qualitative content analysis. Results – Within the context of this research work, various recommendations for action from different areas could be developed. The overriding aim is to promote the rational use of antibiotics. In summary, hygiene measures should be implemented, the general conditions should be adapted, professionals and private individuals should be educated, continuous data collection should be implemented, and diagnostic methods should be adapted. Since responsibility is seen to lie with all those involved, a multidisciplinary approach and the combination of various measures are required above all to achieve a cumulative effect. Antibiotic stewardship has proven to be a suitable tool for this purpose. Conclusion – In the future, comprehensive data on pathogens, resistance and transmission dynamics in dogs and cats must be collected. With the help of these, effective and evidence-based antibiotic stewardship measures can be developed to promote the implementation of rational antibiotic use. The involvement of all stakeholders is essential and reinforces the relevance of the One Health approach. |
