Bedarfsanalyse und Möglichkeit einer Etablierung von Community Health Nurse als Lösungsvariante für die Gestaltung eines multiprofessionell organisierten Primärversorgungsmodells am Beispiel des Stadtteils Hamburg-Wilhelmsburg

Abstract

Mit dem demografischen Wandel steigt der Anteil multimorbider, chronisch erkrankter und pflegebedürftiger Patienten. Die Versorgung chronisch kranker Menschen mit oft komplexen Bedarfen wird in Zukunft, insbesondere in strukturschwachen Regionen, noch mehr an Bedeu-tung gewinnen und nach einer stadtteilbezogenen, koordinierten Verknüpfung von pflegerisch-medizinischen Leistungen verlangen. Das allgemeine Forschungsinteresse der vorliegenden Arbeit richtet sich auf die gesundheitlichen Versorgungsbedarfe der Menschen in einem sozial benachteiligten Stadtteil. Insbesondere wurden die Fragen zur ambulanten Versorgungssitua-tion, zu den Versorgungsbedarfen bei den Teilnehmenden, die Unterstützungsangebote für Pflegebedürftige und pflegende Angehörige und die Bewertung eines CHN-Präventionskon-zeptes in einer Pflegeambulanz im Stadtteil Wilhelmsburg untersucht. Methode Basierend auf einer systematischen Literaturrecherche wurde ein quantitativ orientierter Frage-bogen entwickelt, mit dem Bewohner des Hamburger Stadtteils Wilhelmsburg online sowie schriftlich befragt wurden. Ergebnisse Insgesamt nahmen 208 Einwohner aus Hamburg Wilhelmsburg an der Befragung teil. Die haus-ärztliche primäre Versorgung und besonders die wohnortnahe, fachärztliche Regelversorgung wurden von den meisten (72% der Befragten) als mangelhaft bewertet. Außerdem zeigte sich, dass viele Teilnehmende von komplexen Problemlagen betroffen waren und dass die Lebens-bedingungen eine bedeutende Rolle spielten. Ein CHN-Präventionskonzept in Wilhelmsburg wurde positiv bewertet. Schlussfolgerungen Aus den Ergebnissen dieser Befragung ergeben sich Veränderungsbedarfe, die in die künftigen Planungen des Versorgungsmanagements des Stadtteils einfließen sollten. Diese könnten in weiten Teilen von einem Konzept basierend auf CHN abgedeckt werden.

With demographic change, the proportion of multimorbid, chronically ill patients in need of care is increasing. The care of chronically ill people with often complex needs will become even more important in the future, especially in structurally weak regions, and will require a district-based, coordinated linking of nursing and medical services. The general research in-terest of the present work is directed at the health care needs of people in a socially disadvan-taged urban district. In particular, questions about the outpatient care situation, the care needs of participants, the support services for people in need of care and family caregivers, and the evaluation of a CHN prevention concept in an outpatient care clinic in the Wilhelmsburg dis-trict were investigated. Method Based on a systematic literature research, a quantitative oriented questionnaire was developed, with which residents of the Hamburg district Wilhelmsburg were interviewed online as well as in writing. Results A total of 208 residents from Hamburg Wilhelmsburg participated in the survey. Most (72% of the respondents) rated the primary care and especially the regular specialist care close to home as poor. In addition, it became apparent that many participants were affected by com-plex problem situations and that living conditions played a significant role. A CHN preven-tion concept in Willhelmsburg was positively evaluated. Conclusions From the results of this survey, there are needs for change that should be incorporated into future plans for care management in the district. These could be covered to a large extent by a concept based on CHN.
