Die Schübe äußern sich durch Symptome wie blutige Diarrhoe mit Schleimbeimengungen und Bauchschmerzen. Viele Patient*innen sprechen unzureichend auf die medikamentösen Therapiemöglichkeiten an, was zu anhaltender Symptomatik, eingeschränkter Lebensqualität und Alltagsbeeinträchtigungen führt. Curcumin, eine phenolische Verbindung aus dem Gewürz Kurkuma, wird aufgrund seiner pharmakologischen Eigenschaften bereits seit Jahrhunderten in der traditionellen Medizin Chinas und Indiens eingesetzt. Durch seine antioxidativen und entzündungshemmenden Wirkungen wird sein Einsatz zunehmend auch bei CED diskutiert. Ziel dieser Arbeit ist es, die Auswirkungen von Curcumin auf Entzündungsparameter, Indizes und Scores zur Bewertung der Krankheitsaktivität bei Patient*innen mit CED durch eine systematische Literaturrecherche zu ermitteln. Unter Verwendung spezifischer Schlüsselwörter wurden in PubMed neun relevante Studien identifiziert. Die Ergebnisse zeigen, dass Curcumin in Kombination mit Basismedikamenten das hochsensitive-C-reaktive Protein, das fäkale Calprotectin, die Blutsenkungsgeschwindigkeit, den Colitis Activity Index, den Endoscopic Index, den mucosal appearance score, den Simple Clinical Colitis Activity Index, den Ulcerative Colitis Disease Activity Index, und den partiellen Mayo Clinical Index sowie den Mayo Endoscopic Score bei Colitis ulcerosa verringern kann. Für Morbus Crohn zeigte sich eine Verringerung des Crohn´s Disease Activity Index und des Simple Enodscopic Score for Crohn´s Disease, jedoch bleibt die Datenlage unzureichend. Da keine Studien zur alleinigen Einnahme von Curcumin gefunden werden konnten, müssen die positiven Effekte als Kombinationstherapie betrachtet werden. Zukünftige Studien sollten die alleinige Wirkung von Curcumin, standardisierte Dosierung und Darreichungsformen sowie größere Patient*innenkohorten untersuchen, um die Vergleichbarkeit der Ergebnisse zu gewährleisten und konkrete Empfehlungen abzuleiten. Chronic inflammatory bowel diseases (IBD) are incurable diseases of the intestine characterized by alternating phases of remission and flare-ups. The flare-ups manifest themselves through symptoms such as bloody diarrhea with mucus and abdominal pain. Many patients respond inadequately to drug therapies, leading to persistent symptoms, reduced quality of life, and impairment of daily activities. Curcumin, a phenolic compound found in the spice turmeric, has been used for centuries in traditional Chinese and Indian medicine due to its pharmacological properties. Due to its antioxidant and antiinflammatory effects, its use is also increasingly being discussed in IBD. The aim of this study is to determine the effects of curcumin on inflammation parameters, indices, and scores for assessing disease activity in patients with IBD through a systematic literature review. Nine relevant studies were identified in PubMed using specific keywords. The results show that curcumin in combination with basic medications can reduce highly sensitive C-reactive protein, fecal calprotectin, erythrocyte sedimentation rate, colitis activity index, endoscopic index, mucosal appearance score, simple clinical colitis activity index, ulcerative colitis disease activity index, and the partial Mayo Clinical Index, as well as the Mayo Endoscopic Score in ulcerative colitis. For Crohn's disease, a reduction in the Crohn's Disease Activity Index and the Simple Endoscopic Score for Crohn's Disease was observed, but the data remains insufficient. Since no studies on curcumin alone could be found, the positive effects must be considered as combination therapy. Future studies should investigate the sole effect of curcumin, standardized dosages and dosage forms, and larger patient cohorts to ensure the comparability of results and derive concrete recommendations. |
