dc.description.abstractEinleitung: Shared Decision Making (SDM) beschreibt die gemeinsame Entscheidungsfindung von Ärzt:innen und Patient:innen und gilt als zentrales Instrument einer patientenzentrierten Versorgung. Es fördert sowohl die Versorgungsqualität als auch die Patientensicherheit. Obwohl SDM rechtlich verankert ist, zeigen sich in der stationären Versorgung in Deutschland erhebliche Umsetzungslücken. Ziel dieser Arbeit ist es, die wichtigsten Barrieren bei der Implementierung von SDM zu identifizieren und bestehende Strategien zu deren Überwindung zu analysieren. Methodik: Die Untersuchung erfolgte als systematische Literaturarbeit im Zeitraum von Juli bis August 2025. Eingeschlossen wurden nationale und internationale Publikationen aus den Jahren 1990–2025, die SDM im Kontext der stationären Versorgung thematisieren. Recherchiert wurde hauptsächlich in PubMed und im HAW-Katalog. Die Auswertung erfolgte themengeleitet sowie nach ergänzenden Umsetzungsstrategien. Ergebnisse: Die Analyse zeigt, dass Zeitdruck, ökonomische Fehlanreize, hierarchische Entscheidungsstrukturen sowie Defizite in der interprofessionellen Kommunikation zentrale Barrieren darstellen. Hinzu kommen patient:innenbezogene Faktoren wie geringe Gesundheitskompetenz oder sprachliche Barrieren. Erfolgreiche Strategien umfassen Fort- und Weiterbildungsprogramme, die Integration von Entscheidungshilfen und den Aufbau einer partizipativen Organisationskultur. Praxisbeispiele wie SHARE TO CARE, MAGIC oder „Mit Asklepios nach VORN“ unterstreichen die praktische Relevanz. Diskussion: Die Ergebnisse verdeutlichen, dass rechtliche Rahmenbedingungen allein nicht ausreichen, um SDM nachhaltig zu etablieren. Strukturelle Anpassungen, organisatorische Unterstützung und ein kultureller Wandel innerhalb der Kliniken sind zentrale Voraussetzungen. Fazit: Die Arbeit trägt zum besseren Verständnis der Implementierungsbarrieren bei und leitet praxisorientierte Empfehlungen ab, um Patient:innenbeteiligung und Versorgungsqualität in deutschen Krankenhäusern langfristig zu stärken.de
dc.description.abstractBackground/introduction: Shared decision-making (SDM) refers to the collaborative process in which physicians and patients jointly make healthcare decisions and is considered a key element of patient-centered care. It enhances both quality of care and patient safety. Although legally anchored in Germany, the implementation of SDM inpatient care remains inconsistent. The aim of this thesis was to identify the main barriers to SDM implementation and to analyze existing strategies for overcoming them. Method: The study was conducted as a systematic literature review between July and August 2025. National and international publications from 1990 to 2025 focusing on SDM in inpatient care were included. Searches were carried out mainly in PubMed, and the HAW-Katalog. The analysis was structured thematically and categorized barriers into structural, organizational, professional, and patient-related levels, complemented by identified implementation strategies. Results: The findings highlight that time constraints, financial disincentives, hierarchical structures, and insufficient professional communication are major obstacles to SDM. In addition, patient-related factors such as low health literacy and language barriers, further restrict participation. Effective strategies include targeted training programs, the integration of decision aids, and the development of a participatory organizational culture. Practice-oriented projects such as SHARED TO CARE, MAGIC and the Asklepios-„VORN“-Initiative demonstrate the practical relevance of these approaches. Discussion: The results show that legal frameworks alone are insufficient. To establish SDM sustainably. Structural adjustments, organizational support, and cultural change within hospitals are essential prerequisites. Conclusion: This thesis contributes to a deeper understanding of the barriers to SDM implementation and provides practice-oriented recommendations to strengthen patient participation and quality of care in German hospitals on a long-term basis.en
