dc.description.abstractPhosphat ist ein unverzichtbarer Mineralstoff für den menschlichen Organismus und seinen unterschiedlichen Stoffwechselvorgängen. Es kommt vor allem in proteinreichen Lebensmitteln, wie z.B. Fisch, Fleisch und Milchprodukte, in Form von organischen Phosphatestern, sowie in Getreideprodukten, Gemüse, Hülsenfrüchte und Nüssen als Phytat vor. Zudem werden freie, anorganische Phosphate als Zusatzstoffe in Fertigprodukten wie Tiefkühlware, Gebäck und Schmelzkäse sowie in Erfrischungsgetränken eingesetzt. Aufgrund der Verschiebung der Lebensmittelauswahl sowie die steigende Beliebtheit von Fertigprodukten, die Phosphat-Additive enthalten, ist laut Ergebnissen neuster Studien, in den letzten zwei Jahrzehnten in der USA und Japan eine starke Überversorgung des Mineralstoffes zu verzeichnen. Eine Hyperphosphatämie, die im Laufe der CKD auftreten kann, kommt auch durch zu hohe Phosphataufnahme zustande. Diese kann zu Störung des Knochenaufbaus führen und stellt eine große Morbiditäts- und Mortalitätsgefahr für ESRD Patienten dar. Aktuelle Untersuchungen bestätigen gleiche Risiken, geltend für hochnormale Serum-Phosphatwerte,sowohl für Patienten mit kardiovaskulären Vorbelastungen als auch für gesunde Menschen. Dennoch mangelt es an fundierteren Erkenntnissen zum globalen Phosphatkonsum und dessen Folgen. Aufgrund der Gefahren für die Gesamtbevölkerung besteht Handlungsbedarf. Hier erfordert es die Mitwirkung des Gesetzgebers und der Lebensmittelhersteller mit Hilfe von mehr Transparenz hinsichtlich Inhaltsstoffen und -mengen sowie Verwendung von alternativen Zusätzen gleicher Wirkung. Ein größeres Engagement seitens der Ärzte und Krankenkassen zur Aufklärung über die Konsequenzen zu hohen Phosphatzufuhrs, ermöglicht den Verbraucher bewusstere Kaufentscheidung zugunsten einer phosphatärmeren Ernährung zu treffen.de
dc.description.abstractBeing an indispensable mineral for the human body, phosphate is part of various metabolic processes. It can mainly be found as organic phosphate esters in protein rich foods, such as fish, meat and dairy products yet also in cereal products, vegetables, legumes and nuts as phytate. Additionally, free and organic phosphates are used as additives in convenience products, frozen goods, melted cheese and soft drinks. Due to the shifting towards protein rich foods in the food choice and the rising popularity of finished products, recent studies in the USA and Japan have shown an oversupply of this mineral in the last two decades. The occurence of Hyperphosphatemia in the course of CKD is also caused by a diet high in phosphate. This can disrupt bone mineralization and increases the risk of morbidity and mortality of ESRD patients. Current studies have shown similar hazards for patients suffering from cardiovascular diseases and healthy people with serum phosphate level at high-normal range. However, more profound researches are needed to asses the consequences of high dietary phosphate. Due to the risks concerning the general population, action is demanded. The lawmakers and food producers should aid provinding more transparency on ingridients and their amounts. Moreover, the use of phosphate replacements with similar effects should be encouraged. Finally, enlightments about the consequences of high dietary phosphate load should be given by health insurance funds and doctors in order to achive more awarness regarding foods low in phosphate.en
