Entwicklung einer Patientenbroschüre für die Ernährung im Rahmen einer bariatrischen Operation - Basierend auf aktuellen Empfehlungen und den Ergebnissen einer Patientenumfrage

Abstract

Die Adipositasprävalenz nimmt weltweit zu, allen voran in den Industrieländern. Bislang fehlt es an effektiven Präventionsprogrammen, auch die Langzeitergebnisse von konservativen Therapien verzeichnen sehr geringe Erfolgsraten und können die Krankheit nicht eindämmen. Da Adipositas mit einer Vielzahl von Komorbiditäten einhergeht und eine enorme ökonomische Belastung für das Gesundheitssystem darstellt, muss dringend ein erfolgreiches Therapiekonzept erarbeitet werden. Derzeit stellen bariatrische Operationen die einzig effektiven Therapiemethoden dar, Schlauchmagen- und Magen-Bypass-Operationen werden in Deutschland am häufigsten durchgeführt. Mit einer Adipositas-Operation steigt jedoch auch das Risiko für einen Mangel an Makro- und/oder Mikronährstoffen. Der Erfolg einer bariatrischen Operation hängt von einer guten Vor- und lebenslänglichen Nachbetreuung in einem interdisziplinären Team ab. Ernährungstherapie nimmt hierbei einen großen Stellenwert ein. Es gilt, ein neues Essverhalten mit einer veränderten Lebensmittelauswahl zu erlernen und diätetisch relevante Komplikationen zu vermeiden bzw. zu behandeln. Zurzeit existieren allerdings noch keine Ernährungsempfehlungen mit Leitliniencharakter. Ein weiteres Problem ist die finanzielle Unterstützung der Patienten seitens der Krankenkassen. Untersuchungen von Laborparametern, Ernährungsberatungen oder die Anschaffung von notwendigen Supplementen sind größtenteils Selbstzahlerleistung, die nicht erstattet werden. Da Adipositas mit einem niedrigen sozioökonomischen Status einhergeht, ist diese finanzielle Mehrbelastung ein Problem, gefährdet die Nachsorge und somit den Erfolg einer Adipositasoperation sowie die Gesundheit der Patienten. Ziel dieser Arbeit ist es, die Aufklärung der Patienten zu unterstützen und eine beispielhafte Ernährungsbroschüre zu entwickeln, die im Rahmen der Vorbereitung in Beratungen/Schulungen eingesetzt werden kann. Welche Themen für die Zielgruppe bariatrische Patienten besonders wichtig sind, wurde anhand einer Patientenumfrage an der Schön Klinik Eilbek überprüft und in der Broschüre dementsprechend umgesetzt. Diese Arbeit beschränkt sich auf Operationsmethoden der Sleeve-Gastrectomie und des Roux-Y-Magen-Bypass.

The prevalence of obesity is increasing worldwide, especially in industrialized countries. The success of bariatric surgery depends on a good preparation and a life-long follow-up in a multidisciplinary team. In this case nutritional therapy occupies a large role. It is necessary to learn a new eating behavior with the altered food selection and to avoid nutritionally related complications or treat them. At present, however, there are no guidelines with dietary recommendations in Germany. Another problem is the financial support of the patients by the health insurance companies. Examinations of laboratory parameters, nutritional consultations or the purchase of necessary supplements mainly must be paid by the patients themselves without getting the costs reimbursed. Because obesity is associated with low socio-economic status, this additional financial burden is a problem that endangers the follow-up, the success of an obesity surgery and the health of the patients. The aim of this work is to support the education of patients and to develop an exemplary diet booklet that can be used in the preparation in consultations / trainings. A patient survey at the Schön Klinik Eilbek screened which topics are particularly important for the target group of bariatric patients. These were accordingly implemented in the brochure. This work is limited to surgical methods of sleeve Gastrectomie and Roux-Y gastric bypass.
