Pädiktoren des Einsatzes individueller Gesundheitsleistungen (IGeL) in primärärztlichen Praxen in Deutschland

Abstract

Hintergrund: Die Kassenärztliche Bundesvereinigung (KBV) hat 1998 den Begriff "individuelle Gesundheitsleistungen" (IGeL) eingeführt. Heute haben breite Bevölkerungs-schichten Zugang zu dem Angebot von IGEL. Da bis lang keine repräsentativen Schätzungen über Art und Umfang der angebotenen IGeL vorliegen, soll dieses durch die vorliegende Untersuchung beschrieben sowie Variablen identifiziert werden, die mit dem Angebot im Zusammenhang stehen. Methoden: Die Untersuchung basiert auf der DETECT-Vorstudie, in der repräsentativ für Deutschland Praxischarakteristika und persönliche Merkmale der 3795 befragten Primärärzten ermittelt wurden. Die Vorhersagevariablen für das IGeL-Angebot wurden durch logististsche Regression identifiziert.

Background: In 1998 the Association of Panel Doctors introduced a list of medical services no longer funded by the statutory health insurance ("IGeL-Liste"). The list was criticized for a deversity of reason as to be needless attempt to open rationing, an approach to fleece patients or to undermine self-government etc. Although these services are frequently offered, no exact data on type and quantity of the procedures has been available for so far. This study investigates frueqencies of medical services paid out-of-pocket by the patients as well as the predicting variables for the bargain. Methods: The investigation is based upon the DETECT-prestudy. In a nation-wide recruitment, a representative sample of 3795 primary care doctors were given a questionnaire surveying personal and structural characteristics of each practice. Predictors of out-of-pocket medical services were examined by using logistic regression.
